Correct Care Solutions Discharge Planner in Framingham, Massachusetts
Our Discharge Planner administers a coordinated program of medical and mental health discharge planning, in conjunction with the DOC's transitional planning process pursuant to 103 DOC 493 Reentry Policy, to ensure that each inmate who is in need of continued care at the time of release from custody is linked with appropriate community agencies that will provide such care.
- Bachelor's degree in social work or closely related field from an accredited college or university.
Previous experience in developing successful transition plans involving community resources for persons with mental illness.
Experience in social work case management in a mental health agency for mentally ill population.
- None required.
Attends department release planning meetings and clinical case conferences.
Schedules appointments with community providers to arrange care for acute and chronic illnesses, prenatal services, mental health and substance use disorder services and victims of sexual abuse.
Refers for services in community based medication assisted treatment programs.
Develops release plans to ensure continuity of care.
Refers for service and completes applications for services from the Department of Mental Health (DMH) for those inmates who appear to meet DMH eligibility criteria.
Refers for service and completes application for services from the Department of Developmental Services (DDS) for those Inmates who appear to meet DDS eligibility criteria.
Develops alternative plans for inmates whose DMH applications are denied.
Completes disability application forms for discharged inmates.
Refers inmates in need of mental health services.
Evalutes the need for civil commitment under G.L. c. 123 upon discharge.
Assists inmates with special needs, including, but not limited to, dialysis, mobility impairments, sensory impairments, cognitive impairments, and need for assistive devices (e.g., wheelchairs).
Identifies inmates with medical and mental health conditions who would qualify for MassHealth disability and completion of the Disability Supplement Application.
Identifies housing placement, such as nursing home placement, as needed.
Assists the DOC Correctional Program Officers (CPOs) in securing appropriate housing for inmates identified as having a medical or mental health impairment of such significance as to require special housing needs.
Enters release planning information into the appropriate Inmate Management System (IMS) screens.
Must be able to obtain and maintain security clearance.
Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.
Must be alert at all times; pay close attention to details.
Must be able to work under stress on a regular or continuous basis.
Post orders, if applicable, per site contract.
Perform other duties as assigned.
