Correct Care Solutions Social Worker Supervisor in Indiantown, Florida


Our Continuity of Care Supervisor is responsible for the supervision and operations of the Social Services Department. The Social Services Department acts as the liaison for involvement of the family in recovery planning and serves the primary function of discharge planning by addressing facility discharge barriers. Provides oversight for the discharge process involving step down to civil hospital placement and the community via conditional release plans. Coordination of communication with the Managing Entities, forensic specialists, and community placement liaisons is a primary duty.



  • Master’s degree in Social Work


  • Experience with discharge planning and social security benefits applications.

  • Experience in forensic/correctional/inpatient settings is highly preferred.

  • Two (2) years of experience dealing with severe and persistent mentally ill patients, is necessary.

  • Three (3) years of experience working with the court systems and personnel system, preferred.

  • Discharge planning experience.

  • Working knowledge of the Social Security Benefit application and process.

  • Demonstrated knowledge of the Diagnostic and Statistical Manual of Mental Disorders (DSM-V) and the community resources.

  • Ability to implement intervention strategies based upon analysis of information from verbal, non-verbal, collateral, and narrative resources.

  • Experience utilizing ‘strengths based” clinical practices with individuals and groups.

  • Ability to engage, maintain and appropriately terminate helping relationships with diverse client groups.

  • Excellent written and verbal communication skills.

  • Demonstrated high degree of empathetic listening, tact, and kindness toward others.

  • Ability to work well under pressure, be flexible, be detail oriented, adapt and respond to changing situations.


  • Licensed Clinical Social Worker (LCSW)


  1. Coordinates discharge activities including visits to aftercare facilities, community sites, conference calls with forensic coordinators and specialists, etc. Develops and implements, in conjunction with the unit social worker, interventions to address barriers to the discharge process.

  2. Develops and maintains effective working relationships with state attorneys, public defenders, court liaison offices, forensic specialists and coordinators, community case managers and community mental health center’s supervisory staff. Attends monthly district’s meetings with the Department of Children’s and Family (DCF), Substance Abuse, and Mental Health Coordinators.

  3. Effectively communicates with the treatment team, families, and guardians. Consistently attends and represents clinical needs of residents during community meetings and treatment team meetings. Advocates and negotiates with internal and external statewide systems on behalf of residents consistent with professional conduct and standards.

  4. Conducts professional assessments of structured community placements facilities (aftercare facilities) to determine if they are appropriate for the residents and if they comply with the requirements of the court. Visits placements to determine if they are appropriate for forensic residents. May accompanies residents to court.

  5. Cooperates with colleagues to serve the well-being of residents. Determines when consultation with colleagues is in the best interests of the person’s served, based on best practices consultation.

  6. Maintains and updates medical records in compliance with the hospitals and governmental policies, protocols, guidelines and regulations including DCF and the Joint Commission (TJC).

  7. Participates in the development, implementation, monitoring and evaluation of performance improvement activities with forensic populations. Works to improve hospital service delivery.

  8. Develops discharge plans and conditional release plans in an accurate and thorough manner, in accordance with the hospitals and governmental policies, protocols, guidelines and regulations. Updates all discharge lists as needed.

  9. Continues professional development through educational seminars, workshops, conferences and self-study necessary to maintain current knowledge applicable to the position, including knowledge of the NGI, ITP, Mosher, Baker Act and any other legal commitments.

  10. Is sensitive to cultural diversity issues, treats forensic residents as individuals, and considers the culture of the residents when providing care, treatment and case coordination.

  11. Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.

  12. Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.

  13. Must be alert at all times; pay close attention to details.

  14. Must be able to work under stress on a regular or continuous basis.

  15. Perform other duties as assigned.
